This Rochester, NY not-for-profit agency is seeking a primary care nurse practitioner to provide care for disabled individuals or those with special needs in an outpatient setting.  This agency is committed to maximizing human potentials and inclusion.  You will work 32 hours a week.  You will provide primary health care to enrolled patients in accordance with prevailing standards of medical care and services.  You will participate as a member of the interdisciplinary care planning team and provide timely documentation of delivered care.  Will provide preventative health care and teaching. Promptly records the admission histories, physical exam, orders and plan of care.Within the legal scope of practice of a Nurse Practitioner, you will be responsible for the provision of primary medical services.   You will provide medical evaluations, treatment and medical oversight. This is a direct hire position in which you will work 32 hours a week four days 8am-5pm and on-call every 3-4 weekends with triage responsibility only. Salary 65-70K. Direct hire.
